Borrower Authorization and Certification Form
The Borrower Authorization Form is used to authorize Loans Realty Group by the client to pull their tri-merge (Experian, Transunion, Equifax) mortgage credit report. For husband and wife, they can be both on one single Borrower Authorization Form and for single, they will need to be in separate Borrower Authorization forms. This is an Adobe fillable pdf form that you can type directly onto the form OR you can print and hand write. Please print and wet sign the form (no digital signatures accepted). Please fill out all yellow highlighted sections. Send this form along with a copy of your driver’s license or any government ID that has your picture and signature on it.
Credit Card Authorization Form for Credit Report
This Credit Card Authorization form is to charge the client’s credit card to pull their credit report paid to Universal Credit Services (UCS). UCS is not affiliated with Loans Realty Group. UCS is a 3rd party vendor that we use to access the tri-merge (Experian, Transunion, Equifax) mortgage credit report of the borrower. Cost is $57.35 for single person and $111.70 for husband and wife. This is an Adobe fillable pdf form that you can type directly onto the form OR you can print and hand write. Please print and wet sign the form (no digital signatures accepted). Borrower Authorization and Certification Form MUST be filled out and accompanied with this form.
Credit Card Authorization Form for Appraisal Report
This Credit Card Authorization form for Appraisal Report is to charge the client’s credit card for the appraisal report. We will enter your credit card information into the Appraisal Management Company (AMC) selected by the lender to order your appraisal report. This AMC is not affiliated with Loans Realty Group. Cost of appraisal report depends on type of loan (ie Conventional, FHA), number of units (ie SFR, Duplex, Triplex) and approximate value (ie extra fee if value is over $1M). Cost starts from $525 and above. If the cost of the appraisal report is more than $775, then we will ask for your 2nd approval prior to using your credit card on the AMC’s website.
